✔️ In Section 15.4 of this application, make a note of the time required for you and in Section 15.6 of the Drugs not available at the Government Hospital.  Note that a separate prescription for the day is required, along with the doctor's certificate.
✔️ It is also mandatory to submit a monthly estimate for the government “pharmacy” drug nearest to your residence according to that prescription.
✔️ If the drug is not in the "pharmacy", submit a letter of intent and two estimates for the required medication from two other private institutions.  Please note that requests to transfer the "pharmacy" reserved for the drugs you need to another "pharmacy" will not be considered.  If there is no government pharmacy in the area, the social service officer is required to report it.
✔️ If you are applying for a monthly visit to the clinic, please note that the doctor of the hospital should certify it and the recommendation of the social service officer and the required amount of money once / per month should be certified by the Divisional Secretary.
